This position involves providing individualized and skilled physical therapy services to patients within their homes, aiming to enhance their mobility, reduce pain, and promote overall wellness. The ideal candidate will have a strong background in physical therapy, exceptional interpersonal skills, and a commitment to providing high-quality, personalized care. Conduct initial patient assessments to develop individualized care plans. Implement physical therapy interventions as per the care plan to improve patient's physical function. Educate patients and families on exercises and home care techniques. Regularly evaluate patient progress and update care plans as needed. Maintain accurate and confidential patient records. Collaborate with other healthcare professionals to ensure comprehensive care. Adhere to all professional and safety guidelines set by regulatory bodies. Proficient in various physical therapy techniques and tools. Strong interpersonal and communication skills. Ability to adapt treatment plans to individual patient needs. Good time management and organizational skills. Knowledge of safety protocols and regulations. Medical Insurance Dental Insurance Vision Insurance 401k Current Physical Therapist license. Valid driver's license and reliable transportation. CPR certification preferred.
